Bring in all your medicines, either in their original containers, or make a list of all medication you are taking along with the dosage and frequency. Include both prescription and over the counter, natural and herbal medicines and vitamins. Make a list of all your health questions. Put the questions that are most important at the top of the list. Make a list of all the health care providers you have visited. Write down their names, addresses, phone numbers and the reasons you visited them.
Bring your health insurance card, a photo ID as well as any other insurance information you may need, to your appointment.
Please let us know if you have a hearing, visual or physical impairment, or need an interpreter so that we can accommodate your needs. Feel free to bring along a family member, friend or health advocate to your appointment for assistance in understanding or remembering the doctor’s instructions and give us permission to have your medical condition discussed in their presence.
We accept all major credit cards and personal checks. Please bring personal identification and insurance information to your office visit, and we will notify you if there are changes with the types of insurance we accept. To schedule an appointment, please call during regular business hours at 973-544-8901, by using our online appointment request form, or through your IQHealthâ„¢ portal. Please notify us of any changes in your condition, medications or insurance policy. Please call to notify the office if you are unable to make your scheduled appointment. Failure to show up for your scheduled appointment will result in a $50 charge.
All appointment request dates will be confirmed by office staff based on availability. Submitting a request does not guarantee your appointment.
